Thrive is hiring a Bilingual Care Coordinator

The Care Coordinator is part of the Kansas CARES Project (Coordination and Assistance of Trusted Enrollment Services) and will be responsible for outreach, education, and coverage enrollment for individuals and small businesses in the insurance marketplace, CHIP, Medicaid, Medicare, and other security network services. Additionally, the position will be responsible for helping provide translation services to other community organizations as needed.

Duties and responsibilities

Primary Responsibilities
  • Provide in-person assistance with enrolling or re-enrolling in health insurance, CHIP, Medicaid, and Medicare to individuals and small businesses in the service area.
  • Meet with and educate individuals and small businesses about the types of health insurance programs offered.
  • Help prospective members with complaints, grievances, or questions about their health coverage or a determination related to their coverage or safety net services.
  • Provide information in a fair and unbiased manner that is culturally and linguistically appropriate and accessible to the disabled.
  • Provide transportation services to local residents.
  • Remind potential attendees of appointments through reminder calls and postcards, and follow up on reminder calls and postcards.
  • Comply with requirements set forth by funding sources and Thrive Allen County, including timely submission of project reports and all required data entry.
  • Identify and establish collaboration with organizations and community sites for client recruitment and referral.
  • Serve as a liaison between communities and health care agencies.
  • Advocate for individual and community health by providing counseling and social assistance to residents.
  • Improve residents’ ability to communicate effectively with health care providers.
  • Provide referral and follow-up services.
  • Proactively identify and enroll eligible individuals in federal, state, local, private, or nonprofit health and human services programs.

Ratings

  • Must be fluent in Spanish and English.
  • High school diploma/GED with 2 years of work experience required. Some college education preferred with 1 year of work experience.
  • Ability to use common office software (Word, Excel, Power Point) and cloud-based software.
  • Demonstrated commitment to ensuring access to health care for the medically underserved, especially the poor and uninsured.
  • Excellent verbal and written communication skills in English are required.
  • Independent and good team player.
  • Ability to handle multiple tasks.
  • Ability to work well within the organizational structure.
  • Ability to work with people from all social, economic, racial, and cultural backgrounds.
  • Ability to communicate clearly and effectively in a professional manner.
  • Outgoing, friendly personality who enjoys working with the public.
  • Excellent organizational and decision-making skills.
  • Professional attitude and appearance.

  • Compensation and Benefits: $18.27/hour. Hourly position with medical and dental benefits and 100% of employer paid premiums, IRA with 3% employer contribution, 120 hours of paid vacation per year and 10 paid holidays.
